The Family Health Center: Located at 1730 Chew Street in Allentown and is the original residency practice. It has a mix of patients that includes newborns to those over 100 years old. About 30% plus of the patients are Spanish speaking with approximately half of these who are fluent only in Spanish. Approximately 60% of the patients have Medical Assistance as their primary insurance. This practice is involved in the Pennsylvania Residency PCMH learning collaborative and are recognized as a NCQA designated Level 3 PCMH. Residents from this CCS also provide care to people who live at the Phoebe Nursing Home which is about 4 blocks down Chew Street. Two Resident focused Continuity Care Teams practice here with Dr’s Motley, Sue and Jeff Mathieu, and Jain being the primary faculty on the resident CCT’s.
Lehigh Valley Family Practice Associates: Located in Bethlehem, just off Route 22 and Highway 512. This practice was our first new CCS and has been the practice of two osteopathic physicians for the 20+ years. This is a largely suburban practice with a smaller Medicare, Medicaid, and underinsured population than the FHC. This practice is involved in the LVHN PCMH learning collaborative and are working towards practice transformation. This is the only CCS not currently using Centricity for their medical records and they will be making the transition to electronic medical records over the next two years. The residents assigned to this CCS provide nursing home care to patients of this practice at Manor Care nursing homes. One Resident Continuity Care Team practices here with Dr. Drew Keister being the primary faculty for these CCT.
The Neighborhood Health Centers of Lehigh Valley : Located at 931 Hamilton Street and 143 Linden Street in downtown Allentown. This practice became a CCS in 2009 and has been an outreach site of our Department for the past seven years. The population served by this practice is predominately Spanish Speaking and underinsured. These sites became designated as Federally Qualified “look a like” sites this past year and have applied for a Federally Qualified Health Center grant which is pending at this time. This practice is involved in the Pennsylvania Residency PCMH learning collaborative. The residents assigned to the CCS will provide nursing home care at Phoebe Nursing Home with the FHC residents until they complete their community geriatric needs assessment which will be a focus of activity in the upcoming year led by a graduating resident, Frances Romero. One Resident Continuity Care Team practices here with Dr.’s Abby Letcher and Kristin Reihman serving as the primary faculty for this CCS.
Pleasant Valley: Located in Brodheadsville, about 45 minutes north of Allentown. This practice started as a CCS in March 2010. This practice has an even mix of private and public insured patients. They are the only Medical Assistance providers in Monroe County. They are our most rural practice and are looking to incorporate Telemedicine capabilities into their practice to help bring specialists to their patients and assist in the education of our residents. The population they serve is diverse including farmers, commuters to NY and NJ, variety of ethnic and age groups. The residents assigned to this CCS will provide nursing home care at a nursing home in Brodheadsville. The practice is also involved in the Pennsylvania Chronic Care Governor’s Initiative and are recognized as a level 3 NCQA Patient Centered Medical Home. One Resident Continuity Care Team practices here with Dr. Jon Burke serving as the primary faculty for this CCS.
Bethlehem Medical Center: Located at 2092 Stefko Blvd. in Bethlehem, will be implementing our fifth CCS in July 2011. This practice currently cares for patients that include newborn to those over 100 years old and is creating a special focus in Women’s Health including Low Risk Obstetrics. This CCS will be developing a partnership with LVFPA to provide care to the geriatrics population of both practices, potentially at a nearby assisted living facility. About 40% of the patient population is Spanish speaking with a small percentage who are fluent only in Spanish. Approximately 30% plus of patients are currently enrolled in Medical Assistance as their primary insurance. One Resident Continuity Care Team will practice here with Dr. Holly Binnig serving as the primary faculty for this CCS.
