Community Health Needs Assessment Reports

At Lehigh Valley Health Network (LVHN), it's our mission to heal, comfort and care for the people of our community. Understanding our region's health care needs is an important step in furthering our mission. That is why LVHN is pleased to present the 2016 Community Health Needs Assessment (CHNA) reports for the Lehigh Valley (Lehigh and Northampton counties), Hazleton (Luzerne County), and Schuylkill regions, as well as the 2017 CHNA for Pocono (Monroe County). The reports examine factors that impact the health and wellness of all the people living in these areas. 

Caring for an entire community takes a team of dedicated individuals. That team extends beyond the health care professionals at LVHN. To compile the data in the Lehigh Valley report, multiple organizations comprising the Health Care Council of the Lehigh Valley worked together. The organizations in the council include LVHN, Good Shepherd Rehabilitation Network, KidsPeace, Sacred Heart Healthcare System, St. Luke's University Health Network, the Allentown Health Bureau, the Bethlehem Health Bureau and Neighborhood Health Centers of the Lehigh Valley. 

Creating a healthier community is the focus of all these organizations. By collaborating on the CHNA report, LVHN and these organizations can create a strategy, implement action plans and take meaningful steps in the following priority areas to help make the people of our community healthier and happier.

Priority areas in the Lehigh Valley region:

  • Mental health support
  • Homelessness and substandard housing conditions
  • Low rates of social association
  • High rates of STIs

Priority areas in the Hazleton region:

  • Cancer mortality rates
  • Cardiac mortality rates
  • Diabetes care
  • Improved access to care
  • Low rates of social association

Priority areas in the Pocono region:

  • Obesity
  • Primary Care and Preventive Services
  • Mental Health and Substance Use
  • Smoking

Priority areas in the Schuylkill region:

  • Urgent care services
  • Mammogram screenings
  • Primary/specialty medical care
  • Drug and alcohol abuse
