Lehigh Valley Home Care is a high-quality, nonprofit home health care agency that has been serving the Lehigh Valley and surrounding communities for over 50 years. We are a Medicare-certified, Joint Commission accredited organization. Lehigh Valley Home Care has earned recognition from the American Nurses Credentialing Center’s Magnet Recognition Program®.

Home Care services supports patients in the transition back to a safe level of physical and medical independence in the community and coordinates progression to the next level of care if necessary. Our care model utilizes an interdisciplinary team of health care professionals that includes nurses, home health aides, social workers, physical therapists, occupational therapists and speech therapists. Our staff is focused on helping each patient achieve the maximum potential that is allowable in their home environment by engaging each patient in the treatment process. The home often serves as the best environment for the healing process to occur.

Registered Nurses

Lehigh Valley Home Care seeks highly motivated and skilled nursing staff to join our growing division. We offer a variety of programs to fit the needs of our patients and staff. Full-time, part-time and per diem positions are available as well as a 12-hour weekend program. There is flexibility to move into new roles as the needs of your family or lifestyle may evolve.

Rehabilitation Program

Our approach to in-home rehabilitation is best described as a comprehensive team approach. The treatment team includes physical therapists, occupational therapists and speech therapists as well as rehabilitation and orthopedic registered nurses. Our rehabilitation team provides a multidimensional, holistic approach and individualized plan-of-care designed to meet our goal of a safe transitions in care. We support our therapy staff with clinical leaders who have spent many years as therapists in the home environment. We also provide peer-to- peer consultation with expertise in vestibular, lymphedema and VitalStim® therapies.

Cardiac Program

Our cardiac program specializes in the care of patients with chronic heart failure (CHF), postoperative care after cardiovascular surgery, myocardial infarction, cardiomyopathy, LVAD patients and patients on IV milrinone awaiting heart transplant.

Our highly experienced registered nurses are proficient in managing cardiac disease and provide a program tailored to the in home environment. Our team collaborates with all providers involved in the patients care to ensure a comprehensive approach to achieve positive outcomes.

Behavioral Health Program

The behavioral health team consists of behavioral health nurses, social workers, physical, occupational and speech therapists and home health aides. Our care team will work with families and caregivers to meet our patients specific psychosocial, emotional and medical care needs.

Mother/Baby Pediatric Care Services

Our Mother/Baby Pediatric care team provides services for infants, children and new mothers. Our team provides antepartum education for high-risk patients as well as postpartum care and education including postpartum assessment, breastfeeding support, well-baby assessment and education about well-baby care and development.

Our home care clinicians use a tablet based point-of-care service to communicate with others involved in the care of our patients, giving our care team the most up-to-date and accurate information available.

When indicated, our plan-of-care may include a remote home health monitoring system, providing a link between patients and the health care professionals at Lehigh Valley Home Care. With this advanced system, each patient will have someone watching over them every day; not just during scheduled visits. In addition to providing peace of mind for our patients and their family members, the system allows the patient to actively participate in their care and enables the health care team to make the necessary adjustments to the care plan before problems occur.
