Transcript Requests

A certified official transcript of the Joseph F. McCloskey School of Nursing record is available by clicking here and paying via credit card.

Official transcripts are also available by mail. You may click here to download a transcript request form. Transcripts are available with a written request from the graduate or former student.

The cost of transcripts is $10 (check or money order made payable to "Joseph F. McCloskey School of Nursing").

On the written request, please state your name, address, phone number, year of graduation (or withdrawal), if you had a maiden name at the time of graduating or leaving the program, and where you want the transcript sent. Official transcripts are sent from institution to institution. If you are requesting a transcript for your personal file, the transcript will be marked "unofficial". The Joseph F. McCloskey School of Nursing will not fax transcripts.
