      Registered nurse who engages with patients prior to their first appointment.

      A care management partner (CMP) is a registered nurse who serves as an extension of LVPG primary care practices. In this role, you engage with patients to address concerns prior to the first appointment they have with a primary care provider (PCP). You build relationships with patients on the phone or via video visits to answer questions, learn and document their preferences, and order labs or other items to support a seamless and meaningful first visit with their PCP.

      Registered nurse who focuses on chronic disease management.

      A care navigation partner (CNP) is a registered nurse who serves as an extension of LVPG primary care practices by providing clinical triage and chronic disease management. In this role, you plan and facilitate patient education and address social determinants of health. CNPs are an essential link between patients and their primary care team. You build relationships with patients on the phone or via video visits to provide education, connect them with community resources and provide timely access to care.

      The Registered Nurse (RN) in a physician practice provides care to designated patients within the Pennsylvania Registered Nurse Scope of Practice. Provides care coordination across the continuum. Participates in network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment and electronic medical record system. Performs administrative functions as necessary.

      The Licensed Practice Nurse (LPN) in a physician practice provides care to designated patients within the Pennsylvania Licensed Practical Nurse Scope of Practice. Provides care coordination across the continuum. Participates in network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment and electronic medical record system. Performs administrative functions as necessary.

      Practice support role with responsibility for both clinical and clerical tasks to support practice operations.

      As a medical assistant (MA), you support all functions of an LVPG practice. Your responsibilities include rooming patients, collecting patient history, performing medication refills, administering vaccinations and performing point-of-care testing. You also schedule patient appointments and obtain referrals while supporting patient care. After working as an MA with LVPG for two years, you will be eligible for promotion to medical assistant advanced.

      Student enrolled in a medical assistant program. LVPG covers the cost of school while MA trainee works part time in a practice. Clerical work transitions to clinical work as schooling continues.

      As a medical assistant (MA) trainee, you have an opportunity to pursue or further your education, paid for by LVPG, while you put your skills to work part time in an LVPG practice. Your role is a combination of clerical and clinical functions, with an increasing focus on clinical tasks as you progress in school. When you complete your education, you will be eligible for promotion to medical assistant.

      *Candidates who are hired as MA trainees are required to enroll in an MA program of their choice if they are not already in school.
