LVHN ACO Preferred Partners

Skilled Nursing Facility Tiers

The SNFs designated as Preferred or Aligned have signed a cooperative agreement with LVHN.

SNFs were evaluated based on:

Performance in key quality metrics and patient outcomes

CMS quality ratings and

Readmission rates

Care coordination

Patient volume

location of service and

On-site LVHN/LVPG provider presence

Cost factors

Length of stay

Resource Utilization Group (RUG) and

Cost per episode

Patients will receive information about these partnerships from hospital Case Managers. Patients may choose any SNF, even if not Preferred or Aligned with LVHN. LVPG Geriatrics, through SNF Learning Collaboratives, will continue to work with all facilities.

However, pilots for any new pathways and protocols will be piloted with Preferred facilities and then implemented with all tiered facilities. Evaluations of facilities will occur annually and their relationships with LVHN may change as a result.
