The Department of Community Health engages colleagues, patients and community members in integrating care and promoting health. Community health focuses on the social causes that impact health, such as housing, employment, social supports and access to care. The department’s goal is to advocate for the healthiest community in Pennsylvania – one relationship at a time. The department works hand in hand with LVHN clinical departments, as well as with community members and community-based organizational partners.
What influences our work
The Community Health Needs Assessment (CHNA)
The Department of Community Health represented LVHN on the Health Care Council to conduct the CHNA. Collaboration with other institutions helped the department obtain community input in order to better understand patients’ needs and concerns.
The department also abides by LVHN’s pursuit of the Triple Aim, a framework that aims to deliver better health and better care to community members at a better cost.
Pennsylvania 2-1-1 East
Pennsylvania 2-1-1 East is a free, confidential, non-emergency, comprehensive information and referral service that connects Lehigh County residents with the health and human services they need.
Promise Neighborhoods of the Lehigh Valley - Community Survey
The Promise Neighborhoods of the Lehigh Valley conducts a community survey each year on a rotating basis in its three Promise Neighborhoods to get a better understanding of the residents living in the designated geographical areas. Department of Community Health colleagues are active participants in drafting survey questions, training resident liaisons in data collection, conducting the data analysis, and designing the report that profiles the results each year. The reports are publically available, and the results also are presented to community residents and service organizations annually. Read survey findings for the 2015 surveys.
What we do
Together with community-based partners and strategically aligned departmental partners such as the Air Products and Chemicals Center for Innovation and Connected Care and Population Health, the Department of Community Health’s work is integrated with and supported by data from Populytics. The department welcomes the opportunity for community organizations to partner with any of our programs and initiatives. We aim to:
- Advocate for strategies that improve the social causes of health.
- Develop programs to meet the current and changing health needs of the communities we serve.
- Design ways to measure process and health outcomes to make sure we are effectively and efficiently improving the health and well-being of all our community members.
- Provide consultation for LVHN community-related activities including:
- Community Benefit Committee
- Veterans Affairs Initiative
- School-based health clinics
- Medical practices and clinics
