Health Advocacy Program (HAP)

HAP is an initiative that helps meet the social and economic needs of patients from the Lehigh Valley Physician Group (LVPG) Community Practices at Lehigh Valley Hospital (LVH)–17th Street. This program trains college students to become health advocates and connects patients to services that will meet the basic needs that affect their health outcomes. Some of these outcomes include:

  • Asthma exacerbations
  • Depressive symptoms and other psychosocial measures
  • Housing stability
  • Food insecurity

 Internships are being established with local colleges so students can receive college credits for volunteering.  

What is the goal? 
HAP’s health advocates meet patients’ basic needs, such as food, housing, clothing and transportation, to help improve patients’ social and economic needs that affect health outcomes. This allows increased understanding of the challenges people with low incomes face in getting or keeping healthy
