Remote patient monitoring (RPM)
For patients with a chronic disease, such as congestive heart failure, remote patient monitoring lets patients reside at home with the comfort that their condition is being monitored every day. By detecting subtle changes in patients’ vital sign trends, visits to the emergency department or re-hospitalizations are reduced or prevented.
- If a patient receives home care services, the RPM program augments their care; it does not replace scheduled home care visits.
- Patients with an LVPG Primary Care Provider or Cardiologist are eligible for this program.
