Enrolling in Benefits

  • All benefit-eligible employees (.4 FTE or higher) will have the opportunity to enroll in their benefits after their start date.
  • You will be required to enroll online within 30 days from your date of hire.
  • Your benefits will be effective the first of the month following 30 days of employment, unless otherwise indicated by your recruiter or credentialing coordinator.
  • Once you have reviewed the briefing and have made your final decisions, you will be able to proceed with your online enrollment during your first week of employment. This must be done from an LVHN computer.
  • During the online enrollment process, you will need to submit the appropriate and applicable dependent documentation. Please have the following document in electronic format ready before you start the online enrollment process:
    • Marriage license for spousal coverage
    • Birth certificates for child coverage
  • Additional information you will need to enroll:
    • Dependents Social Security numbers
    • Information about additional coverage your or your dependents may have
  • A Benefits Counselor will be at Connections to answer your questions.
