Criminal Clearances

All students completing a clinical rotation or internship must complete criminal background checks and submit to LVHN's Office of Student Affairs via their respective CastleBranch Bridges or VSLO accounts prior to beginning their educational experience at LVHN, pursuant to Human Resources Policy #1002.90 “Criminal Background Checks.” The process for completing these clearances is as follows:

  1. Student completes (Candidate's Affidavit for Provisional Employment/Participation (CPSL) and submits to LVHN Office of Student Affairs.
  2. Student completes online request for Pennsylvania State Police Criminal History Report (PATCH form). The purpose of this check is: Employment/Screening.
  3. Student completes Child Abuse History Clearance (CAHC) Application. Students can view/print report directly from PA child welfare site. (Please select application purpose: “14 years of age or older…”)
  4. Student requests Department of Human Services FBI Fingerprint Clearance, consistent with the Child Protective Services Law. Please click here for instructions.
    ALL MEDICAL AND PHYSICIAN ASSISTANT STUDENTS ONLY:
    If you are NOT currently a Pennsylvania resident for the past two years, you are also required to submit a second set of fingerprints to the Pennsylvania Department of Aging with the Older Adults Protective Services Act (OAPSA).
  5. ALL OTHER STUDENTS: Students under consideration for the Transitional Skills Unit (TSU) Home Care and Hospice, or for departments floating to TSU (click here for listing) who have not been a Pennsylvania resident for the past two years are also required to submit fingerprints to the Department of Aging with the Older Adults Protective Services Act (OAPSA).
  6. GRADUATE NURSING STUDENTS ONLY must also submit to LVHN Office of Student Affairs:
    1. Copy of RN license
    2. Copy of professional liability insurance.
