The following agencies and resources aid and support individuals to live in the community.

In your local community, Findhelp.com locates resources that assist with finances, transportation, utilities, food and meals, housing (dial 211) and other social needs.

You can apply online at ssa.gov/ssi or in person at your local Social Security office. Tri-County Advocacy Project (877-774-6380) can assist with completing the forms.

A program that delivers healthy hot meals to the homes of elderly, sick or other people who have a hard time making meals for themselves. Additional Meals on Wheels programs can be found at mealsonwheelsamerica.org.

Adult day care is a facility that provides supervision, meals and activities for adults who cannot safely be left alone at home during the day. These centers are often used as respite by family members caring for a loved one. Private duty home care can provide many services from medical and nursing care to helping with transportation and doing paperwork (such as paying the bills) and many other things that will help older, sick or disabled people to live independently. Please visit pahomecare.org for a list of home health providers in Pennsylvania.

For Medicaid insurance plan coverage, contact member services (number on back of ID card) to see if you are eligible for any transportation services/coverage.

Medicare: Nonemergent transportation through an ambulance company for medical appointments and treatments. These services are available to individuals able to walk without assistance. Not available through all ambulance companies, need to contact your local service. Phone: 610-395-6833.

Uberhealth: A website and app that can assist with transportation to medical appointments and treatments.

Agencies to assist families/caregivers for options in the community.

Many families become the caregivers for loved ones so they may continue to live in their own home. This can be very difficult, and many times the caregiver finds they need a short break. Taking a short break (also called respite care) allows the caregiver to take better care of him/herself. Caregivers who take a break and can take better care of themselves are often more successful at caring for their loved one. The following is a list of places that can give you more information about places and services that provide respite care:

Personal Care Homes

A facility that provides shelter, meals and supervision. Please visit dhs.pa.gov for a complete list of personal care homes in Pennsylvania.

Assisted Living Facilities

A facility that provides residents daily care and activities of daily living. There are different levels of care.

Skilled Nursing Facilities

A skilled nursing facility is defined as an institution (or a distinct part of an institution) which serves as a home that provides medical or nursing care. Please visit sais.health.pa.gov for a complete list of skilled nursing facilities in Pennsylvania.
