Summary

Provides mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members who present themselves for psychiatric evaluation with a broad range of mental health needs. Coordinates care for clients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment.

Job Duties

  • Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
  • Utilizes management activity to include authorization of services as well as management of denial and appeal process.
  • Provides case management and care coordination services for patients.
  • Intercedes on behalf of individual patients to assist them in achieving and maintaining community stability. Acts as a liaison between patients, referral sources, and funders.
  • Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.
  • Manages all elements of mental health voluntary and involuntary commitments processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs).

Minimum Qualifications
  • Bachelor’s Degree Social Work, Psychology, or related field and
  • Actively enrolled in a related Masters degree program.
  • Ability to remain patient and calm in stressful situations.
  • Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.
  • Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive care plan.

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
