At LVPG Anticoagulation Management Services, we help patients safely manage their warfarin (Coumadin) therapy. Our team is led by a nurse practitioner and medical director and includes nurses and medical assistants who have undergone specialized training.
To be enrolled in the program, you need to be referred by either your specialist or primary care doctor.
