SummaryThe Care Progression Manager collaborates across interdisciplinary teams to advance health system strategies, including initiatives focused on value-based care and payer population management. Partners with clinical, operational, and financial stakeholders to align care progression workflows with organizational goals for quality, efficiency, and cost-effectiveness. The Care Progression Manager progresses patients care based upon medical criteria for in-patient status.
Job Duties
- Oversee population to ensure timely progression of care, safe discharge planning, and appropriate post-acute services using evidence-based criteria.
- Identifies patient clinical conditions, testing and value based clinical interventions that can be completed in the
ambulatory space with appropriate follow-up.
- Apply clinical criteria to monitor medical necessity, reduce avoidable days, and resolve barriers to care in collaboration with physicians, nursing, and utilization management.
- Collaborates in the development and implementation of performance improvement activities in collaboration with
care management, physician, utilization management.
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Monitor length of stay, readmissions, and throughput metrics; identify trends; and contribute to initiatives that improve efficiency and patient outcomes.
Minimum Qualifications- Bachelor’s Degree in nursing.
- 5 years experience in-patient acute care hospital.
- Excellent communication (oral and written) required to facilitate reviews and discussions with providers.
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire or
- RN - Licensed Registered Nurse_NJ - State of New Jersey Upon Hire
Preferred Qualifications- Master’s Degree in nursing.
- Knowledge of care and utilization management
- Strong interpersonal and organizational skills to manage case load and communicate with multi-disciplinary team
members.
- Ability to work independently and set priorities to ensure appropriate use of hospital resources.
- Ability to lead individuals and groups.
Physical DemandsLift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
