Tips to help streamline the registration process
Your hospital services start in the Patient Registration Department. Our Patient Registration Staff will require certain information from you including your demographic and insurance information. They will ask you for this information even if you have been a patient here prior. Please have this information ready and provide updated primary and secondary insurance information, at every visit. Also, be sure that you have the most current insurance cards with you. The Registration Staff will obtain a copy of identification from you also. You will be asked to sign a consent form for services as well as other necessary forms. If you have Medicare insurance it is required by Medicare that a Medicare Secondary Payer Form be completed at every visit. If you are not sure if your insurance will cover certain services or tests please contact your insurance, You will be responsible for deductibles, co-payments, co-insurance or non-covered services amounts that your insurance company determines is your responsibility.
