Keep Your COVID-19 Vaccination Records at Your Fingertips With MyLVHN

MyLVHN makes it easy to keep your COVID-19 vaccination records and test results accessible.

A new question people may face during the pandemic is if you’ve received a COVID-19 vaccination or have been tested for COVID-19. Many places, like restaurants and even Broadway shows, require proof of vaccination before you enter. MyLVHN, Lehigh Valley Health Network’s (LVHN) patient portal powered by Epic, makes it easy to keep your COVID-19 vaccination records and test results accessible.

You can share your information quickly, easily and securely by creating a QR code right from MyLVHN. You also can export your COVID-19 information to another app.

Generate a QR code with your COVID-19 information

  • Log in to your MyLVHN account.
  • From the menu, choose MyRecord, then choose COVID-19.
  • To generate a bar code that can be scanned, click QR Codes. Please note, if you have two COVID-19 vaccinations and test results on file, you will have separate QR codes for each one. You can choose what you would like others to view.

Export your COVID-19 information to another app

  • Log in to your MyLVHN account.
  • From the menu, choose MyRecord, COVID-19.
  • Select Download/Export.
    • If you are using the MyLVHN mobile app, a series of screens appears to help you verify that you have another app installed on your device into which you can export your information. Then, another screen appears to show the data you can share. If you have more than one COVID-19 vaccine and test results, a checkbox appears for each one. Select the box for the information you would like to share with the other app, and then click Export. Select the app into which you want to export your data and follow the instructions in that app.
    • If you are using the MyLVHN website: If you have more than one COVID-19 vaccination and test results on file, a checkbox appears for each one. Select the checkbox for the information you would like to share through the other app and then click Export. A copy of your information is downloaded to your computer device. To import it to another app, go to that app on a mobile device and follow the app’s instructions.

Print a PDF

You also can print a PDF that states when and where you received your COVID-19 vaccine, as well as which vaccine you received. Just log in to your MyLVHN account and then from the menu choose MyRecord, COVID-19 and then select Download/Export.
