Lehigh Valley Health Network’s (LVHN) COVID-19 Assess and Test Center at 140 Sherman Court, Hazleton, will close effective January 13. Anyone experiencing COVID-19 symptoms can receive testing at Lehigh Valley Hospital (LVH)–Hazleton laboratory, 700 E. Broad St., Hazleton. Lab hours are 6:30 a.m. to 3 p.m., seven days a week. A medical order (also known as a script) is required to receive a COVID-19 test.

“COVID-19 is still prevalent in our community. We must remain vigilant and continue to decrease the spread of infection by washing our hands, covering our mouths when coughing or sneezing, wearing masks in indoor public spaces when ill, and following a recent COVID-19 exposure or positive COVID-19 test.” - Ammar Abbasi, MD

If you think you have COVID-19 or may have been exposed to someone who does, stay home. Take an at-home screening test or use one of the following options:

  • Contact your primary care provider: Call or visit MyLVHN.org or open your MyLVHN app. Tap on the message, select “Send a message” and follow the prompts.
  • Nurse line: 888-402-LVHN (5846), option 4
  • LVHN ExpressCARE Video Visit: Visit MyLVHN.org or open your MyLVHN app. Tap on the menu, then select “Schedule ExpressCARE Video Visit.”
  • Complete an E-Visit (detailed questionnaire visit submitted to a provider) by visiting MyLVHN.org or your MyLVHN app. Tap on the menu, then select “E-Visits."

During the screening, a medical professional will determine if you have COVID-19 symptoms and if you should receive testing. If needed, you will be directed to a COVID-19 testing location, such as the LVH–Hazleton outpatient lab. When symptomatic or following a recent COVID-19 infection, you must have a medical order (also known as a script) in order to receive a COVID-19 test at the LVH–Hazleton location. Your insurance will be billed for the test.
