LVH Diabetes and Metabolism Center-Tower Place

At LVPG Diabetes and Metabolism Center–Tower Place our team provides comprehensive care and education services to adults with diabetes and prediabetes, as well as nutritional support for those with obesity, chronic kidney disease and other metabolic conditions.

What we offer:

• Education – Topics covered include healthy eating, exercise, medication management, risk reduction and stress management.
• Care companion – Diabetes educational resources through your MyLVHN account.
• Medical nutrition therapy – Individual nutrition counseling for your condition.
• Diabetes educator visits
• Lifestyle management – You will have the opportunity to meet with a lifestyle medicine clinician as well as work with our health coaches.
• Medication optimization – If you have uncontrolled diabetes and enroll in this program, you can work with one of our clinicians to optimize your diabetes medications. 
• Remote patient monitoring
• Prediabetes management – LVHN can help people with prediabetes learn how to delay or prevent the onset of type 2 diabetes.
