LVHN Occupational Medicine offers a range of services for employees in the transportation industry. 

Department of Transportation (DOT) Physical Exam

The Department of Transportation (DOT) requires a specific physical exam to help certify that drivers do not have any physical, mental, organic or other condition which might affect their ability to safely operate a commercial motor vehicle.

The exam includes:

  • Physical exam (including review of medical history, neck measurement, body mass index [BMI] reading, and vital signs)
  • Vision screen (with horizontal field of vision)
  • Hearing screening
  • Urinalysis 

DOT requires the physician to conduct a thorough investigation into concerns such as cardiac disease, diabetes, hypertension, sleep apnea or other disorders that need to be addressed before the medical examiner’s certificate is issued. 

All medical examiners have been certified by the Federal Motor Carrier Safety Administration (FMCSA) and are listed with the FMCSA National Registry. 

Substance Abuse Testing

LVHN Occupational Medicine locations provide Substance Abuse and Mental Health Services Administration (SAMSHA)-approved urine testing for marijuana, cocaine, opiates, amphetamines and phencyclidine (PCP) for pre-placement, post-accident, random, reasonable suspicion and return-to-duty. Breath alcohol testing is also available. All controlled substances and alcohol testing procedures are conducted in accordance with DOT administering regulations requiring alcohol and/or drug testing. Learn more. 

DOT Random Testing Consortium or Individual Company Pool

LVHN Occupational Medicine offers DOT-regulated employers the opportunity to participate in random drug testing either as an individual company or as part of a consortium with other companies in order to meet Title 49 Code of Federal Regulations’ random urine controlled substances and alcohol testing regulations. The pools ensure your compliance with requirements for random testing. Occupational Medicine’s administration office maintains and administers the program and provides the required statistical summaries. An annual maintenance and administration fee is charged per participating random testing employee.                                     

Reasonable suspicion training

LVHN Occupational Medicine can provide training to supervisory level employees to identify the signs leading to reasonable suspicion of drug or alcohol use in the workplace.
