That's why health centers at Jefferson Health are strategically located in the heart of the communities they serve, close to the places people visit every day, including shopping centers, grocery stores, and other essential destinations. These convenient locations make it easy for you to access care when you and your family need it most.

In recognition of National Health Center Week, Aug. 2-8, we're shining a spotlight on the important role health centers play in connecting people with the care, services, and support they need to live healthier lives.

Healthcare Connections in the Community

Health centers provide comprehensive care under one roof. You can conveniently access a variety of services, including primary care, specialty care, laboratory services, imaging, rehabilitation, and at many locations, JeffExpress Urgent Care for walk-in treatment of minor illnesses and injuries.

These centers are more than healthcare facilities. They are community access points that promote health awareness, wellness, education, and safety. Last year, health centers hosted nearly 200 community events, helping strengthen connections between healthcare professionals and the communities they serve.

A Front Door to Care and a Bridge Home

Health centers play an essential role throughout a patient's healthcare journey. For many people, they serve as a first point of entry into Jefferson Health through primary care visits and specialty consultations. When additional treatment or hospitalization is needed, patients can be seamlessly connected to hospital-based services.

The connection works in both directions.

After a hospital stay, health centers help patients continue their recovery close to home. Follow-up appointments, rehabilitation, diagnostic testing, and specialty care are available throughout the region, reducing travel and making it easier for patients to stay engaged in their care plans. Jefferson Health specialists regularly travel to health centers across the region, increasing access to expert care and helping patients receive many services without traveling to a hospital campus.

By the Numbers

The impact of health centers can be seen across our area:

  • 58 health centers serve communities in the Lehigh Valley, eastern Pennsylvania, and southern New Jersey.
  • More than 3 million patient visits occurred at these health centers over the past 12 months.
  • More than 6,000 clinicians and healthcare professionals provide care in these locations.

Expanding Access Across the Region

Jefferson Health continues to bring healthcare closer to where people live and work. Last summer, two new facilities opened at Tower Place in Bethlehem, Pa.

Women's Health Center at Tower Place is designed specifically around women's healthcare needs. It offers thoughtful amenities such as an on-site café and childcare services for patients with children while they receive care. The center also brings together a comprehensive range of women's services.

Health Center at Tower Place provides a wide variety of services in one convenient location, including primary and specialty care, JeffExpress Urgent Care, rehabilitation, and much more. 

Opened in January 2026, Health Center at Pocono Creek offers comprehensive care for individuals and families. Services include lab, family medicine, cardiology, outpatient adult rehabilitation, vascular surgery, and cardiac and vascular diagnostic testing.

Looking Ahead

Jefferson Health's commitment to community-based healthcare continues to grow. The Children and Family Health Center, scheduled to open in Hanover Township in early 2027, will provide general pediatrics, Children's ExpressCARE, as well as pediatric surgery, pulmonology, gastroenterology, and rehabilitation. 

In addition, construction starts soon on Upper Providence Health Center in Royersford, Pa. It will bring JeffExpress Urgent Care, primary care, Toll Heart and Vascular Institute, imaging, and lab services to this rapidly growing area.

A Shared Vision for the Future

The concept of one-stop, patient-centered healthcare is reflected across Jefferson Health. The Honickman Center in Center City Philadelphia brings together nationally ranked specialty programs, advanced technology, and patient-focused design in a single location. From cancer and respiratory care to gastroenterology and otolaryngology, the facility demonstrates how integrated care can create a more seamless healthcare experience.
