Summary

Functions as part of the clinical team providing clinical care under the direction of an LPN, RN, or provider. Assists with administrative, clinical, and clerical duties which could include taking a patient's history and personal information, scheduling appointments, responding to patient communications, and rooming patients.

Job Duties

  • Prepares patient and equipment for exams/procedures and maintains exam rooms and clinical area.
  • Performs medical history collection and medication refill, per protocol.
  • Collects and prepares lab specimens/lab requisitions accurately and schedules and obtains authorizations for referrals and/or procedures.
  • Identifies actual or potential patient risks and communicates those risks to the licensed nurse or provider upon recognition.
  • Adheres to clerical, clinical, and EMR workflows.
  • Performs clerical duties and provides front office support duties such as greeting and welcoming patients and providing excellent customer service both in-person and over the phone.
  • Collects co-payments, schedules appointments, updates missing patient demographics, obtains referrals and pre-certifications.
  • Uses patient centered care concepts in delivering care and performing related activities.

Minimum Qualifications
  • Specialized Diploma as a Medical Assistant
  • Ability to accept and adapt to constant change within a healthcare setting.
  • Ability to take initiative and function independently as well as collaboratively.
  • Excellent computer skills including EMR, word processing, and Excel.
  • American Heart Association Basic Life Support - State of Pennsylvania within 30 Days

Preferred Qualifications
  • Associate’s Degree from a Medical Assistant Program
  • CCMA - Certified Clinical Medical Assistant - NHA - State of Pennsylvania or
  • CMA - Certified Medical Assistant - AAMA - State of Pennsylvania or
  • CMAC - Clinical Medical Assistant Certification - AMCA - State of Pennsylvania or

Patient Care Coordinator - Neighborhood Hospitals - Full Time

Requires working between LVH- Macungie and Gilbertsville Neighborhood Hospitals (working two 12 hr. shifts (7a - 730p) in the Emergency Dept. and two 8 hr. shifts per week.

Summary
Ensures and/or provides patient care in accordance with the RN job description. With the Director, assumes accountability for the direction and management of patient care within assigned area. Assists Director in the management of material, financial, and human resources within the assigned unit.

Job Duties

  • Oversees administrative and operational issues for the designated care delivery team.
  • Functions as a department team leader to support unit and hospital goals and objectives.
  • Uses effective and appropriate communication styles with patients, significant others, visitors, staff, and professional colleagues.
  • Supports the Culture of Safety by following established policies and procedures which include identification and timely patient safety event reporting.
  • Participates in coaching, counseling, and mentoring of unit personnel.
  • Participates in completing performance appraisal of team members.
  • Supports the Professional Practice Model and participates in efforts related to the key components.
  • Assumes unit, clinical, and administrative responsibilities in the absence of the Director with Administration approval and under the supervision of the Patient Care Services management staff.
  • Ensures and/or provides patient care in accordance with all responsibilities in the RN job description.
  • Ensures staffing to meet patient care needs and operational guidelines.
  • Fosters accountability of staff for adherence to policies/procedures/professional practice.
  • Ensures completion of the unit's schedule in a timely and equitable manner.
  • Utilizes evidence based research findings within patient care delivery, communicates research findings to co-workers and/or the professional community, attends research continuing education offerings, assists with data collections, and serves as a participant in ongoing research studies.
  • Provides clinical, consultative, technical, or informational services as needed to staff, other disciplines, patients, and/or families.


Minimum Qualifications

  • Bachelor’s Degree Nursing
  • 3 years Registered nurse in an acute care hospital setting.
  • Ability to quickly learn and adapt to new technologies, processes, and ideas.
  • Ability to teach and direct others with positivity and encouragement.
  • Demonstrated proficiency in directing and guiding personnel.
  • Strong analytical, critical thinking, and problem-solving skills.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire

Summary

Evaluates, plans, directs, and administers programs of Occupational Therapy (OT) and plans and conducts OT programs to restore function, prevent disability, and help patients reach their maximum level of independence.   • Establishes and implements appropriate occupational therapy plans of care to restore function prevent disability and help patients reach their maximum performance level.
  • Implements an individualized treatment plan through the use of skilled evidence based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, splinting/DME, and modalities.
  • Provides occupational therapy treatment consistent with the patient's needs assessment.
  • Performs reassessment when indicated and modifies Occupational Therapy plans accordingly.
  • Communicates patient progress and physical limitations to a supervisor, physician, and other team members.
  • Implements an occupational therapy plan of care to individuals based on age specific and population specific needs utilizing a developmental and patient family directed approach.
  • Provides supervision in accordance with regulatory requirements, as appropriate.
  • Establishes an Occupational Therapy treatment plan with measurable goals and time frames.
  • Provides cost effective care in accordance with departmental billing guidelines.
  • Completes and submits all documentation based on policies/standards and regulatory requirements.

Minimum Qualifications

Summary

Directs and manages the care of designated operative patients and performs an ongoing patient assessment, creates and maintains a safe environment, and implements and enforces all policies and procedures. Handles the instruments, supplies, and equipment necessary during the surgical procedure.   • Assists operating physician with surgical tasks such as homeostasis, suturing and wound exposure as well as patient positioning.
  • Contributes to the overall care of the patient before, during and after surgery by using technical and mechanical functions.
  • Performs circulating functions such as monitoring, recording, and communicating patient condition and managing overall nursing care of the patient before, during, and after the procedure.
  • Performs scrub functions such as the selection and handling of instruments and supplies used during procedure.
  • Recognizes the need for and initiates collaboration/communication with physicians, interdisciplinary team members, and takes actions to proactively resolve patient needs.
  • Inspects sterile items for integrity before opening or delivery to the sterile field and verifies the presence and appropriate color change of chemical indicators in all loads.
  • Collects data pertinent to the healthcare consumer's health or the situation and analyzes the assessment data to determine actual or potential diagnoses, problems, and issues.
  • Controls environment including temperature, provision of warm blankets, use of warm irrigation fluids, monitoring of the sensory environment, control traffic flow, and adheres to safety and sanitation policies.

Minimum Qualifications
  • Specialized Diploma Nursing
  • Less than 1 year Current operating room experience or completion of perioperative internship program.
  • Ability to perform assessments and nursing functions as acquired in the basic nursing preparation program.

Summary

Evaluates, plans, directs, and administers physical therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence. Provides assessment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning them to their prior level of function. Improves and/or facilitates quality of life through prescribed exercise, hands-on care, and patient education. Helps people achieve fitness goals, regain or maintain their independence, and lead active lives.

Job Duties

  • Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
  • Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
  • Provides physical therapy treatment consistent with the patients' needs assessment.
  • Performs reassessment when indicated and modifies physical therapy plans accordingly.
  • Communicates patient progress and physical limitations to supervisor, physician(s), and other team members
  • Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.
  • Provides supervision in accordance with regulatory requirements, as appropriate.
  • Establishes a physical therapy treatment plan with measurable goals and time frames.
  • Provides cost effective care in accordance with departmental billing guidelines.
  • Completes and submits all documentation based on policies/standards and regulatory requirements.

Minimum Qualifications
  • Bachelor’s Degree Physical Therapy
  • Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
  • Knowledge of physical therapy techniques.
  • Must be able to provide safe, compassionate, and competent physical therapy treatments.
  • Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire

Summary

Plans and implements the total palliative care of the patient in support of the prescribed healthcare team plan. Provides direct nursing care with special emphasis on controlling pain and other symptoms of the hospice patient.

Job Duties

  • Maintains appropriate standards from OSHA regarding contact with, and possible exposure to, bloodborne pathogens and other potentially infectious body materials.
  • Assumes responsibility for the appropriate assessment, planning, intervention, evaluation, management, and referral related to patient health conditions.
  • Provides preventive health services to facilitate students’ optimal physical, mental, emotional, and social growth and development.
  • Assesses the physical and psychosocial needs of students and provides direct health services or referrals to address acute and chronic conditions.
  • Designs, develops, and communicates clear, effective health education messages in oral and written formats for individuals and groups.
  • Provides health counseling/classes and education to patients and family members on nutrition, disease prevention, chronic diseases, and healthy lifestyles.
  • Observes, records, and reports to the physician and/or team leader the patient's signs and symptoms, response to treatment, and changes in the patient's condition.
  • Coordinates presentations by various agencies and professionals on pertinent health care topics.
  • Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.

Minimum Qualifications
  • Graduate of accredited Nursing program
  • 1 year of nursing experience.
  • Ability to handle difficult and stressful situations with professional composure.
  • Knowledge of home care or hospice conditions.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire

Position Overview

This is a rewarding part time Dentist opportunity for an existing clinician to pick up additional hours aiding the pediatric community. Candidate will need to have a passion for providing care to school childern or YMCAs. the Mobile unit will travel to areas from from Lehigh Valley to Minersville, Jim Thorpe, or Lehighton.

 

In a Pediatric Dentist career with LVHN, you’ll experience:

  • Providing preventive and restorative dental care on the new Miles for Smiles mobile unit that will be arriving in June/July 2024. 
  • Collaborating with mobile staff team and work with the dential medicien staff of 55 Dentist, 21 clinical staff, and 8 residents. 
  • A mobile staff of a hygienist, mobile driver/registration specialist, and dental assistant. 
  • Seeking a dentist open to work when available; Seeking a dentist available as a per diem and can fill in as needed. 
  • Pediatric patients needing screening exams from local schools and YMCA facilities.
