Summary

Provides triage telephone service to ensure prompt identification of patients with high-risk conditions. Provides telephonic triage in order to assist callers in determining the most appropriate level of care. Facilitates care coordinator and virtual monitoring services within identified business areas and follows workflows, utilizes protocols, and engages resources to provide supportive service to patients and customers.

Job Duties

  • Directs callers to appropriate level of care and provides healthcare advice/education using assessment skills, approved guidelines and protocols, and knowledge of self-care practices.
  • Assists in all aspects of medical record maintenance and accurately files all patient charts and reports in the designated location in patient records.
  • Provides clinical support in the assessment of symptoms, determination of symptom acuity, offering information related to symptom management, and patient education.
  • Facilitates coordination of care across settings for a designated population ensuring that labs, studies, and referrals are completed and results are discussed with the patient.
  • Performs a short evaluation of the patient situation to estimate the severity of illness and/or injury that includes chief complaint, vital sign, and mental status information.
  • Prepares documentation information and transmits medical information.
  • Serves as a liaison between customers, patients, physicians, other members of the public and hospital staff, programs, and other resources.
  • Refills prescriptions for patients and places orders for appropriate testing and/or labs.

Minimum Qualifications
  • Associate’s Degree in Nursing.
  • 5 years experience in nursing (acute care, ED, medical office, telephone triage) and
  • 2 years previous clinical acute care hospital experience/urgent care.
  • Colleagues working within the department who advance from LPN to RN with a minimum of 2 years experience within the department are eligible.
  • Excellent computer skills and concerned empathy for individuals seeking healthcare-related information and assistance.
  • Flexibility to perform a variety of related and unrelated tasks throughout the day and work in a fast paced environment.
  • Knowledge of appropriate home health information for patients to follow until visit with physician, if they do not need an immediate ED intervention.
  • Knowledge of telephone-based clinical assessment techniques and medical practice telephone triage protocols.
  • Must exercise judgement in dealing with sensitive, confidential information.
  • Skill in making triage decisions and responding quickly and calmly in emergency situations.

Summary

Obtains benefits and authorizations for surgical procedures, diagnostic testing, medications, outgoing referrals, and other services as part of daily operations. Determines the authorization protocols for each health plan and performs billing duties to ensure proper and timely payment is received from insurance carriers and patients.

Job Duties

  • Collaborates with physicians and provider office staff in ascertaining the appropriate authorization based on medical necessity and the treatment plan provided.
  • Performs a medical necessity check to determine if procedure and diagnosis support medical necessity.
  • Ensures authorizations are obtained in accordance to network policy (any authorization not falling into policy guidelines is communicated to ordering office, patient, and manager).
  • Communicates direct/indirect with insurance companies to obtain insurance verification, benefits and precertification for approval.
  • Verifies additional clinical information and insurance authorizations/referrals.
  • Reviews and monitors WQs/schedules to ensure that proper and accurate authorization has been received prior to patient’s visit.
  • Maintains compliance with benchmark data regarding accounts registered versus scheduled procedures.
  • Determines estimated patient financial responsibility using insurance verification information and payer contracts and/or self-pay guidelines.

Minimum Qualifications
  • High School Diploma/GED with specialized training in insurance, coding, billing, or similar healthcare certificate programs.
  • 1 year in a healthcare setting with insurance verification/authorizations.
  • Familiarity with billing procedures and payer reimbursement.
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Proficient in utilization management processes, standards, and managed care.
  • Proficient in standard medical practices and insurance benefit structures.

Preferred Qualifications
  • Associate’s Degree

Summary

Evaluates, plans, directs, and administers physical therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence. Provides assessment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning them to their prior level of function. Improves or facilitates quality of life through prescribed exercise, hands-on care, and patient education. Helps people achieve fitness goals, regain or maintain their independence, and lead active lives.

Job Duties

  • Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
  • Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
  • Provides physical therapy treatment consistent with the patients' needs assessment.
  • Performs reassessment when indicated and modifies physical therapy plans accordingly.
  • Communicates patient progress and physical limitations to supervisor, physician(s), and other team members.
  • Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.
  • Provides supervision in accordance with regulatory requirements, as appropriate.
  • Establishes a physical therapy treatment plan with measurable goals and time frames.
  • Provides cost effective care in accordance with departmental billing guidelines.
  • Completes and submits all documentation based on policies/standards and regulatory requirements.

Minimum Qualifications
  • Bachelor’s Degree Physical Therapy
  • Ability to work at various locations per diem.
  • Knowledge of best practices in physical therapy.
  • Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
  • Must be able to provide safe, compassionate, and competent physical therapy treatments.
  • Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.

Summary

Responsible for clerical duties within the rehabilitation department to support smooth and organized office operations including; answering phones, paitent check-in, ordering/maintaining office supplies, record keeping, and data entry/maintenance. These individuals will also be responsible for assist the rehab staff in preparation of patients and/or the environment of care for therapy sessions. This individuals, under the supervision of a licensed therapist, can also assist in safe patient handling. They will also be expected to help maintain departmental cleanliness, therapeutic supply inventory and other duties as assigned by the licensed therapy staff and/or next level manager.

Job Duties

  • Assists in transfers and stand by assistance of a licensed therapist during skilled interventions.
  • Assists Physical Therapists, Occupational Therapists, Speech Therapists, and/or Exercise Physiologists by retrieving needed equipment, setting up the environment of care prior to treatments, cleaning equipment, and maintaining department supplies.
  • Assists with rehabilitation department clerical tasks
  • Transports patients to and from department or room via wheelchair or stretcher.
  • Performs regular sweeps of all patient areas to locate ambulatory assistive devices and returns them to their proper location.

Minimum Qualifications
  • High School Diploma/GED
  • Less than 1 year
  • Ability to work in a fast-paced environment.
  • Ability to understand and follow instructions.
  • Strong word processing skills
  • Ability to work under pressure to meet deadlines.
  • Strong customer service orientation.

Summary

Provides quality food service in accordance with all state and federal regulations and meets the nutritional and individual needs of each patient. Assists in the menu selection, service, and delivery of food for the patient population served. Follows and maintains union policies and procedures throughout the scope of work.

Job Duties

  • Assists in preparation of supplies and setting up of tray-line area prior to each meal.
  • Delivers food promptly and professionally maintains safe food handling and sanitation standards.
  • Engage with patients in a friendly and professional manner while actively listening to their concerns
  • Follows meal patterns and evaluates patient meal according to approved diet manual.
  • Keeps work area clean and orderly.
  • Knowledgeable about the food and drinks that is served and can handle various dietary requirements.
  • Participates in the quality assurance, mealtime assessment, and environmental assessment.
  • Protect confidential information based on HIPPA and other regulatory privacy laws
  • Take orders for meals, print meal receipts, meal count, and submit to cook
  • Work closely with all Registered Dieticians to anticipate patient needs.

Minimum Qualifications
  • 1 year of experience in the Food Service Industry
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation.
  • Ability to interpret and adhere to union policies and procedures.
  • Ability to read and understand instructions and diet order cards.
  • Coordination and dexterity for preparing food and operating dietary equipment safely.

Summary

Coordinates and provides a variety of clerical/administrative duties associated with daily physician practice operations. Serves as the initial point of contact for patients and healthcare providers within the practice. Delivers the high standard of customer service necessary to maintain the overall patient experience.

  • Keeps abreast of changes in all insurance providers regarding coverage and reimbursement changes.
  • Organizes effectively and efficiently to accommodate interruptions and changing priorities.
  • Performs scheduling, scanning, billing, filing, referrals, work queues, and other clerical functions in a timely manner.
  • Performs appropriate patient outreach include telephone and EMR messaging.

Minimum Qualifications
