You may need copies of your medical records to share with a physician or for insurance or legal purposes.

- Complete the Authorization for Release form of Protected Information.
- Once you complete the form, there are directions on how you can submit the form.

Lehigh Valley Health Network works with a release of information vendor, MRO, to coordinate providing copies of medical records to patients and authorized representatives. If you would like to check the status of a previously submitted request, please contact MRO directly at 610-994-7500.
Health Information Exchanges
LVHN may make your health information available electronically through a state, regional or national Health Information Exchange (HIE) service.
What is a Health Information Exchange (HIE)?
An HIE is a secure, electronic network that allows your healthcare clinicians to instantly look up your medical history, test results, and prescriptions to coordinate your care. Because many states and medical networks automatically include your records in an HIE by default, you must explicitly "opt out" if you want to stop this electronic sharing.
What happens if you opt out?
- Your clinicians cannot search your records. Doctors, nurses, and specialists treating you will not be able to pull up your health information through the HIE.
- It may lead to potential delays and repeat tests. Opting out can slow down communication between your doctors. If you go to a new doctor or an emergency room, they may not have immediate access to your medication lists or recent lab results, which could lead to duplicate tests or delays in treatment.
How do I opt out or back in?
- Complete the Request to Opt Out of Health Information Exchanges form and return it to the Health Information Management (HIM) department.
- If you would like to reverse your choice and “opt back in,” please complete the Revocation of Opt-Out in Health Information Exchanges form and submit it to the HIM department.
