Tips to help streamline the registration process
Your hospital services start in the patient registration department. Our patient registration staff will require certain information from you including your demographic and insurance information. They will ask you for this information even if you have been a patient here prior.
When you present at registration, please have this information ready and provide updated primary and secondary insurance information, at every visit.
The registration department will obtain a copy of identification from you. You will also be asked to sign a consent for services as well as other necessary forms.
If you have medicare insurance, it is a requirement of medicare that a medicare secondary payer form be completed at every visit.
If you are not sure if your insurance will cover your services or tests, please contact your insurance. You will be responsible for deductibles, co-insurance, co-payments or non-covered services that your insurance determines are your responsibility.
