Provides quality pharmaceutical care, and fills/dispenses medications in accordance with the latest pharmacy standard practices. Reviews prescriptions and medication orders for appropriateness and compatibility with current therapy, disease states, drug allergies, adverse drug effects, duplication of therapy and compliance to ensure patients' medication therapy is safe, effective, timely, and free from drug-related problems. Communicates with all healthcare professionals and patients concerning medical care to ensure safety and effectiveness. • Prepares and dispenses medications for infant, pediatric, adolescent, adult, and geriatric patients.
• Evaluates evidence-based literature to offer appropriate clinical recommendations in order to optimize drug selection and individualization of therapy.
• Ensures and executes policies and procedures that ensure appropriate, cost-effective, and safe medication use.
• Performs clinical medication surveillance, documents interventions, reports adverse drug reactions and medication errors.
• Coordinates the activities of pharmacy technicians engaged in the preparation, labeling, and delivery of medications and other pharmaceutical supplies.
• Counsels/Provides drug information pertaining to medications, drug usage, and compatibility to physicians, nurses, other healthcare professionals, and patients in accordance with OBRA or other regulatory standards.
• Weighs, measures, and prepares sterile and non-sterile medication products utilizing appropriate tools and equipment.
• Acts as a resource or consultant to other healthcare professionals regarding appropriateness, availability, cost, and/or alternativeness to therapies.

Summary

Job Duties

• Assesses, plans, implements, and evaluates the care for designated patients. • Functions as a department team member to support unit and hospital goals and objectives. Participates in all phases of the performance improvement process, including organizational and unit-based patient experience goals.
• Performs validated technical skills based upon clinical specialty and unit practice.
• Utilizes technology and incorporates technology to optimize alternative modes of care delivery.
• Demonstrates leadership and team building skills including: - Teaching and mentoring new colleagues and department interns. Additional precepting, as assigned. - Delegating on the basis of skills and expertise of team member, acuity and specific needs of the patient, and time availability of team members. - Providing input as requested for performance appraisals of care delivery team members.

• Graduate of accredited Nursing program.
