Summary

The Institute Care Continuum Manager (ICCM) oversees the continuum of care for patients, community partners, and contract stakeholders for ease of access to our Institute and other network services for the right care at the right place by the right provider. The ICCM will also be responsible to develop and implement access pathways into services and will define and monitor quality metrics for this care delivery to ensure goals and objectives are being met. The ICCM will serve on the Institute leadership team to provide insight into barriers to patient access and work with the operational and clinical leadership to remove barriers and create streamlined pathways for care. The ICCM will oversee the development and execution of access operation improvement strategies including referral pathways both internal and external, practice workflows, creating capacity, and innovation techniques. This will include managing institutional provider templates, daily scrubbing of schedules, and the ownership of access which will require identifying and creating open slots for utilization purposes.

Job Duties

  • Oversees the coordination and scheduling of complex appointments for patients across the continuum of care including office visits, diagnostic testing, and other ancillary services that require coordination of multiple resources and timely access to services.
  • Manages and identifies access for the institution while partnering with leadership for improved outcomes.
  • Develops new, innovative and efficient documentation and communication platforms for providers, including platforms to use in non-traditional settings.
  • Facilitates communication amongst the physician/provider, and clinical care team as well as financial, outpatient, ancillary, and supportive services.
  • Develops innovative patient engagement applications and communication avenues to provide streamlined access to expert care.
  • Develop tracking and metrics to quantify the value of the institute and works collaboratively with the departmental team to gather, summarize and report data on performance.
  • Facilitates cross-training and education programs for staff.
  • Directs, supervises, and appraises personnel and ensures the delivery of quality workflows.

Minimum Qualifications
  • Bachelor’s Degree in related field.
  • 2 years experience interacting with patients throughout the continuum of care and
  • 3 years experience working within a complex health system environment or related field.
  • High degree of accuracy and attention to detail.
  • Enhances skills through continuing education.
  • Ability to handle numerous tasks at the same time.
  • Superior customer service skills.
  • Working knowledge of basic computer functionality.
  • Effective and professional written and verbal communication.
  • Ability to handle patients and families in stressful situations.
  • Ability to maintain strict confidentiality.
  • Ability to adapt to changing priorities.
  • Critical thinking skills and solutions-focused approach.

Preferred Qualifications
  • Master’s Degree Master's Degree in health care administration, MBA, or related field.
  • Knowledge and experience with electronic medical records and scheduling software.
  • Knowledge of medical terminology.

Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

PHYSICAL THERAPIST (PT) - HOME CARE POCONOS - FULL TIME DAYS

Summary
Evaluates, plans, directs, and administers physical therapy programs to restore function, prevent disability, and help patients reach their maximum level of independence. Provides assessment of patient condition based on diagnosis from the referring physician. Develops and updates an appropriate plan of care in collaboration with the patient/family to assist in returning them to their prior level of function. Improves and/or facilitates quality of life through prescribed exercise, hands-on care, and patient education. Helps people achieve fitness goals, regain or maintain their independence, and lead active lives.

Job Duties

  • Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
  • Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
  • Provides physical therapy treatment consistent with the patients' needs assessment.
  • Performs reassessment when indicated and modifies physical therapy plans accordingly.
  • Communicates patient progress and physical limitations to supervisor, physician(s), and other team members
  • Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.
  • Provides supervision in accordance with regulatory requirements, as appropriate.
  • Establishes a physical therapy treatment plan with measurable goals and time frames.
  • Provides cost effective care in accordance with departmental billing guidelines.
  • Completes and submits all documentation based on policies/standards and regulatory requirements.


Minimum Qualifications

  • Bachelor’s Degree Physical Therapy
  • Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
  • Knowledge of physical therapy techniques.
  • Must be able to provide safe, compassionate, and competent physical therapy treatments.
  • Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.
  • Ability to travel to patient care assignments - Home Care only
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire


Preferred Qualifications

  • Master’s Degree Physical Therapy or
  • Doctorate Physical Therapy
  • 1 year clinical experience for Home Care


Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Summary
Provides quality pharmaceutical care, and fills/dispenses medications in accordance with the latest pharmacy standard practices. Reviews prescriptions and medication orders for appropriateness and compatibility with current therapy, disease states, drug allergies, adverse drug effects, duplication of therapy and compliance to ensure patients' medication therapy is safe, effective, timely, and free from drug-related problems. Communicates with all healthcare professionals and patients concerning medical care to ensure safety and effectiveness. Provides direction to technical staff and participates in performance enhancement initiatives.

Job Duties

  • Prepares and dispenses medications for infant, pediatric, adolescent, adult, and geriatric patients.
  • Evaluates evidence-based literature to offer appropriate clinical recommendations in order to optimize drug selection and individualization of therapy.
  • Ensures and executes policies and procedures that ensure appropriate, cost-effective, and safe medication use.
  • Performs clinical medication surveillance, documents interventions, reports adverse drug reactions and medication errors.
  • Coordinates the activities of pharmacy technicians engaged in the preparation, labeling, and delivery of medications and other pharmaceutical supplies.
  • Counsels/Provides drug information pertaining to medications, drug usage, and compatibility to physicians, nurses, other healthcare professionals, and patients in accordance with OBRA or other regulatory standards.
  • Weighs, measures, and prepares sterile and non-sterile medication products utilizing appropriate tools and equipment.
  • Acts as a resource or consultant to other healthcare professionals regarding appropriateness, availability, cost, and/or alternativeness to therapies.


Minimum Qualifications

  • Doctorate Pharmacy or
  • Bachelor’s Degree Pharmacy
  • 2 years relevant pharmacist experience or
  • 1 year PGY1 pharmacy residency
  • Ability to accurately dispense and monitor medications.
  • Ability to work effectively in a team environment.
  • Ability to make independent judgments under stressful situations at times.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • RP - Registered Pharmacist - State of Pennsylvania Upon Hire


Preferred Qualifications

Summary

Provides direct patient care functions associated with the general maintenance of patients well being including obtaining items as requested by patients supervising licensed clinician, patients movements within their care environment and walking with appropriate patients under the direction of licensed care providers. Assist patients in preparing for therapy and therapeutic activities. Provides other support services, dictated by patient needs, as a care delivery team member. Utilizes effective and appropriate communication styles. Supports unit and hospital goals and objectives.

Job Duties

  • Provides care related activities for designated patients which is directed by RN, PT, OT, ST, EP and/or recreational therapist using a patient/family centered approach.
  • Reports observations/changes in patient status to the appropriate nursing and/or medical personnel and performs hourly purposeful rounding to assess pain, toileting, and positioning needs. Reports any issues or abnormalities.
  • Assists with transfer, gait, and daily living. Assists with preparing patients to go to therapy and at times will assist with transport to therapy.
  • Assists patients with dysphasia and ensures dining club adherence.
  • Assumes responsibility for and completes routine assignments for a designated group of patients within the allotted period of time.
  • Functions as a department team member to support unit and hospital goals and objectives.
  • Ensures patient care guidelines are followed in order to reduce and prevent self harm.
  • Completes placement of restraints, chair/bed alarms, and intakes and outputs (I&O) documentation.

Minimum Qualifications
  • High School Diploma/GED
  • Less than 1 year Patient care experience or customer service experience
  • Ability to recognize and act upon changes in patient condition.
  • Ability to understand the marketplace and changing needs of clients with proactive solutions that target these needs.
  • Ability to work a flexible schedule outside of the normal business hours to meet the needs of patients.
  • Demonstrates clinical knowledge of rehab admission criteria, leading to appropriate indentification of qualified rehab patients.
  • Competent in monitoring and reporting basic vital sign monitoring including HR, BP, RR
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire

Summary

Performs with latitude for independent judgment, ingenuity and initiative, ultrasound and/or vascular exams of designated anatomy to physician specifications and established protocols. May participate in call rotations, as necessary.

Job Duties

  • Assists in non-routine specialized exams, such as vascular studies and procedures utilizing advanced imaging equipment and techniques.
  • Performs diagnostic ultrasound procedures on various vascular systems of the body for diagnosis, treatment, and prevention.
  • Ensures quality control and quality improvement activities according to guidelines and federal and state regulations.
  • Maintains current level of expertise in topics related to ultrasound and participates in the appropriate level of continuing education required.
  • Coordinates in establishing proper parameters and protocols for imaging equipment in conjunction with radiologists.
  • Assists in revising and reviewing current protocols to ensure the highest quality of diagnostic tests.
  • Ensures that appropriate consents are present and signed before taking patient from waiting room.
  • Produces diagnostic images and processes and prepares images for interpretation by the radiologist.

Minimum Qualifications
  • Specialized Diploma Graduate of approved CAHEA school of diagnostic medical sonography or certification program. In good standing with the ARDMS.
  • Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
  • Ability to assess performance of yourself, other individuals, or organizations to make improvements or take corrective action.

Summary
Provides diagnostic quality radiologic imaging under the direction of a Physician while maintaining proper radiation protection standards. Follows the guidelines for radiation safety while imaging gently. Delivers a high level of patient care and customer service to promote patient satisfaction. Works in a hospital facility that offers full hospital services. Provides direct patient care to high acuity patients encompassing emergency, inpatient and outpatient classes supporting up to and including trauma and ICU.



Job Duties

  • Produces images of diagnostic quality, processes, and prepares images for interpretation by the radiologist.
  • Provides radiation protection in accordance with prescribed safety standards. Uses appropriate exposure factors, collimation, and proper distance with documentation. Assumes responsibility and accountability for his/her own safety and the safety of others.
  • Ensures appropriate exam is ordered for number of images provided and attached to the correct patient folder.
  • Confirms levels of supplies and equipment to meet requirements for patient care and/or specific departments.
  • Documents LMP, fluoro time, radiologist, number of images, appropriate history and all other required documentation.
  • Ensures coordination of daily work duties within the team and coordinates timing of exams.
  • Ensures images have crossed into PACS before study is complete.
  • Confirms physician orders are complete and includes signature, ordered study, reason for test (diagnosis/CPT codes) and obtains missing information prior to treatment.
  • Reports equipment malfunction and the need for adjustment and/or repairs.
  • Assists in the education and training of new employees, students, and other ancillary personnel.
  • Assumes responsibility for cleanliness and sanitation of the immediate work area and maintains orderliness.


Minimum Qualifications

  • Technical School Diploma Graduate of accredited Radiography program or equivalent
  • Less than 1 year Completion of Radiologic program
  • Ability to deal effectively and ethically with medical and confidential information relating to patients, family, and physicians.
  • Ability to recognize, analyze, and solve a variety of problems.
  • Understanding of how to effectively and safely use radiation producing equipment.
  • Knowledge of medical technology and terminology.
