Patient-centered medical homes (PCMHs) are similar to our community care teams (CCTs). In both, health professionals work as a team to better manage your care and prevent potential health problems.

Our PCMH aligns with our CCT service to empower the patient. We help you to not only comply with your doctor’s recommendations, including medications, but also to take extra steps toward better health.

A medical home improves care for chronic disease

Your PCMH care team takes the time to get to know you and the complexity of the chronic disease you are managing. They go the extra mile to ensure you receive care that makes sense for you and your lifestyle.

PCMH teams deliver patient-centered care

Your care team will do more than go through the motions of a regular check-up – they will anticipate your needs and take actions based on your illness(es).

For example, we know our patients with diabetes will require eye exams. Rather than waiting for patients to make an appointment, our team updates the medical record and proactively places the order for an eye exam.

Our service extends beyond clinical care. We offer education to help you better understand your disease. We can even arrange transportation to doctors’ appointments for those who need it.

Medical homes help with lifestyle and behavior modification

To successfully manage a chronic disease, you need to be dedicated to improving your health, which may include:

  • Losing weight
  • Changing your diet to add more fruits, vegetables and whole grains
  • Quitting smoking
  • Increasing exercise

These efforts require a commitment from you – but you don’t need to go it alone. Your team can identify resources to support you in whatever changes you must make to live a healthier lifestyle.  

Patient-centered support for medications and medical equipment

If you need medical devices like walkers, we can help arrange for you to get them. And, if you were hospitalized and given medicines that aren’t available from your insurer after discharge, we’ll educate you on other ways to get your medications covered.
