How to enroll in MyLVHN

With an activation code (Find your activation code on the After Visit Summary form you receive at check out from an LVPG or Coordinated Health office visit.)
- Go to MyLVHN.org or download the MyLVHN app.
- Select "Sign Up Now" button
- Complete the online form. You will need your personal activation code and last four digits of your Social Security number.
Without an activation code
- Visit MyLVHN.org or download MyLVHN app.
- Select “Sign Up Now” button
- Select “Sign Up Online” to complete the online form. You will need to provide your Social Security number. If available, include your LVHN medical record number.
During an LVPG or Coordinated Health office visit
You can also activate your account during checkout following a visit with your LVPG or Coordinated Health provider.
Proxy access information
If you are a parent or guardian to a child or another person, request proxy access at their next LVPG or Coordinated Health appointment. Proxy access lets you schedule appointments, check immunization records and set up LVHN Video Visits on their behalf. In order to set up proxy access, you will also need your own MyLVHN account.
Tip: If you see your child’s or loved one’s photo on your MyLVHN account, you have proxy access.
Out of town care
With MyLVHN, you have access to your medical history and almost all of the portal’s convenient features wherever you are – whether at home or on the road. (Note: To have a Video Visit, you must be in the state of Pennsylvania to receive care due to medical licensing requirements.)
If you are out of town and need to access medical care, your electronic health record can be made available to an authorized health care organization, helping to ensure you receive safe and comprehensive care. And should you receive care at another health care provider, you can link your MyLVHN account to that other provider and then have the ability to see all of your health information in one place. Go to “Account Settings” in MyLVHN and select, “Link My Accounts.”
