Castle Branch/Bridges System
For schools utilizing the Castle Branch/Bridges System: In order to be approved to begin, all requirements must be completed and submitted to LVHN via the Castle Branch/Bridges system at least two weeks prior to the beginning of the student experience. If required documentation is not submitted at least two weeks prior, the student start date will be delayed until all required documentation is complete.
For schools still PENDING: Please complete the forms for your hospital location (below) until you are completely set-up in Castle Branch/Bridges system. In order to be approved to begin, all requirements must be completed and submitted to LVHN at least two weeks prior to the beginning of the student experience. If required documentation is not mailed at least two weeks prior, the student start date will be delayed until all required documentation is complete.
Overview and Orientation
Please select the location where you will be learning to access your orientation and all forms you will need to submit prior to your start date. If you are a current employee, you only need to submit the Employee Exemption Form.
