Summary

Transports patients and assists with direct patient care and procedures that have been ordered by a physician. Performs guest assistance duties such as escorting patients and assisting clinical staff with transferring patients from bed to transport equipment.

Job Duties

  • Transports patients to and from hospital departments for radiology testing.
  • Assists in transport of patients eligible for discharge.
  • Uses the hospital telephone system as it relates to transport tracking in order to accept transport orders.
  • Assists nurses and technologists in moving patients to and from bed, stretcher, and/or wheelchair.
  • Provides directional information to guests and visitors and patient location to patient's family members and guests.
  • Helps patients change their clothes, as requested.
  • Retrieves, cleans, and stores all unused patient transport equipment.
  • Supplies the department with clean linen and takes soiled linen to the laundry.
  • Replaces oxygen tanks, as required.

Minimum Qualifications
  • High School Diploma/GED
  • Less than 1 year
  • Ability to work independently with minimal supervision.
  • Ability to multi-task and take initiative.
  • Ability to exercise sound judgment in making critical decisions.
  • Strong interpersonal skills and ability to build positive relationships.

Summary

Participates in the collaborative identification and reporting of patient safety issues. Provides continuous observation or constant roving to observe and intervene when risky patient behaviors are identified. Communicates with the patients to deescalate tense situations, verbal communication with care team members, and documents in the patients electronic medial record in regular intervals. Performs or assist patients with ADL's, meals, and ambulates patients. Ensures that the patient environment is safe and reports any safety hazards to the nurse. Provides continuous, direct, one-to-one patient visual and audible observation of patients of all ages to maintain a safe and therapeutic environment.

  • Takes initiative in compassionately rendering services or responding to needs and interacts hospitably, cheerfully, and supportively with staff, patients, visitors, and work colleagues.
  • Seeks help or advice as soon as possible when patient appears to be in medical distress or poses a threat, and escalates to assigned medical professionals.
  • Assists patient with ordering dietary tray and setting up of patient's meal tray and assists with basic positioning of patient.
  • Participates in hand-off of pertinent information/behavior about assigned patient when arriving on unit and upon completion of shift assignment.
  • Assists with patient wheelchair rides and/or ambulation if the patient is stable and gait has been previously determined safe.
  • Assists in completing specific direct patient care tasks.
  • Responds rapidly and thoroughly to emergencies, and special needs.
  • Provides continuous direct one-to-one patient observation.
  • Documents observations in the patient's electronic medical record in regular intervals

Minimum Qualifications
  • High School Diploma/GED
  • Ability to recognize and report data reflective of the patients' status and identify each patient's specific needs.
  • Ability to walk or stand for prolonged periods.
  • Knowledge of the principles of growth and development through the life span.
  • Knowledge of department policies and procedures.
  • Therapeutic Crisis Intervention Certification - State of Pennsylvania Upon Hire
  • American Heart Association Basic Life Support - State of Pennsylvania within 30 Days

Summary

Functions as part of the clinical team providing clinical care under the direction of an LPN, RN, or provider. Assists with administrative, clinical, and clerical duties which could include taking a patient's history and personal information, scheduling appointments, responding to patient communications, and rooming patients.

  • Prepares patient and equipment for exams/procedures and maintains exam rooms and clinical area.
  • Performs medical history collection and medication refill, per protocol.
  • Collects and prepares lab specimens/lab requisitions accurately and schedules and obtains authorizations for referrals and/or procedures.
  • Identifies actual or potential patient risks and communicates those risks to the licensed nurse or provider upon recognition.
  • Adheres to clerical, clinical, and EMR workflows.

  • Collects co-payments, schedules appointments, updates missing patient demographics, obtains referrals and pre-certifications.

Summary

Assists with the daily operations of the clinical staff, procedures, treatment, and support of the physician's office health care team. Delivers/oversees care provided to patients and functions as an integral member of the leadership team focusing on clinical support staff. Handles general administrative tasks such as scheduling, reception, front end revenue cycle, medical assistant duties, and other physician office tasks. Serves as a clinical resource for all practice staff and is accountable for clinical staff orientation, education, clinical skills competency, clinical office operations, and clinical skills remediation. Facilitates seamless transitions and completion of initiatives designed to enhance the patient care experience inclusive of pre-visit planning and clinical intelligence tools across the continuum.

Job Duties

  • Oversees clinical operations and facilitates patient engagement through practice settings including visit flow, clinical triage, EMR clinical intelligence tools, pre-visit planning, transitions of care follow-up, and other patient focused initiatives to enhance the patient care experience.
  • Assumes clinical and administrative responsibilities in the absence of the Director/Manager.
  • Completes end-of-day tasks including running applicable reports, identifying no-show patients and communicating with appropriate personnel, completing daily batching tasks, and other office close procedures.
  • Prepares daily charts and maintains medical files and electronic medical records.
  • Obtains clarification of policy, whenever necessary, and may use resources available through other departments.
  • Assesses clinical staff competencies and provides remediation, as necessary, in collaboration with Clinical Services.
  • Facilitates coordination of care across settings for a designated population and ensures that labs, studies, and referrals are completed and results are discussed with the patient.
  • Provides regular feedback and involves other colleagues in quality improvement workflow development.
  • Reviews clinical staff competencies and provides remediation, as necessary.
  • Fosters accountability of staff for adherence to policies/procedures/professional practice.
  • Participates in and applies research in practice and supports the practice and hospital goals and objectives.

Minimum Qualifications
  • Associate’s Degree from an Accredited School of Nursing as an RN
  • 3 years Recent experience in a clinical setting

Serves as a clinical resource for all practice staff and is accountable for clinical staff orientation, education, clinical skills competency, clinical office operations, and clinical skills remediation. Facilitates seamless transitions and completion of initiatives designed to enhance the patient care experience inclusive of pre-visit planning and clinical intelligence tools across the continuum. &lt;br&gt;&lt;br&gt;&lt;b&gt;Job Duties&lt;/b&gt;&lt;ul&gt;&lt;li&gt;Oversees clinical operations and facilitates patient engagement through practice settings including visit flow, clinical triage, EMR clinical intelligence tools, pre-visit planning, transitions of care follow-up, and other patient focused initiatives to enhance the patient care experience.&lt;/li&gt;

Summary

Oversees the LVPG Central Education Team and the Float Pool Teams to ensure safe and effective delivery of patient care and services. Oversees performance management of education programs, regulatory clinical expectations, development and implementation of all education, consultation services for aligned network groups, and region-specific team development. Directs all aspects of LVPG Central Education financial budgeting and accounting management. Manages the nursing and clinical support team members in multiple locations and collaborates with the directors, leaders, and administrators. Manages clinical-support decision-making, clinical workflows, and clinical staff supervision. Oversees clinical performance and quality improvement activities in the practice including development and implementation of protocols and standardization of work. Serves as a clinical resource and role model for nursing, clinical support team members, clerical staff, and the practice management team.

Job Duties

  • Facilitates meetings with clinical educators regarding the assessment of practices and clinical staff to ensure necessary planning and development of education.
  • Oversees strategies for development, implementation, and evaluation of annual competencies to ensure the development of clinical staff.
  • Establishes team performance objectives and metrics.
  • Oversees and manages the administrative, human resources, and operations of the education department.
  • Develops, evaluates, monitors, and adjusts the budget to ensure that expenses are sufficient to support the department and remain within set budget.
  • Educates and clarifies concepts related to initiatives and programs for team members and clinical staff.
  • Maintains and summarizes data for presentations and reports in a clear, concise format to colleagues and the executive team.

Minimum Qualifications
  • Master’s Degree in healthcare, education, nursing, and/or business.
  • 5 years of experience in healthcare management or education.
  • Demonstrates knowledge of the problem-solving process in the identification and resolution of operational, educational, or other problems.
  • Demonstrates excellent problem solving and reasoning skills.
  • Demonstrated proficiency in directing and guiding personnel.
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Summary

Provides professional nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation in accordance with facility or department policies and procedures. Summary
